ABSTRACT

OBJECTIVE:To eva luate the risk of abno rmal female reproductive system haemorrhage induced by novel oral anticoagulants (NOACs). METHODS :The abnormal female reproductive system haemorrhage reports induced by 4 kinds of NOACs as “dabigatran etexilate ”,“rivaroxaban”,“apixaban”and“edoxaban”were used as the first suspected dugs to collected from FDA adverse event reporting system (FAERS)database during Jan. 1st,2004-May 31st,2019. The report odd ratio (ROR) method was used to detect the signal of abnormal female reproductive system haemorrhage induced by NOACs. RESULTS :A total of 2 658 adverse events related to abnormal female reproductive system haemorrhage were collected from FAERS database , involving 330 reports of dabigatran etexilate ,2 049 reports of rivaroxaban ,267 reports of apixaban ,and 12 reports of edoxaban. The abnormal female reproductive system haemorrhage caused by dabigatran etexilate ,apixaban and edoxaban mainly occurred in patients aged 75 and older ,accounting for 37.27%,36.70% and 58.33% respectively;that of rivaroxaban mainly occurred in patients with 45-64 years old ,accounting for 33.04%. The incidence of severe adverse events (SAE)induced by dabigatran etexilate,rivaroxaban,apixaban and edoxaban were 96.36%,84.53%,47.19% and 58.33%,respectively. All of patients in the included reports were mainly hospitalized and hospitalization stay wa s prolonged ,accounting for 64.78%,90.01%,86.51% and 71.43% ,respectively. A total of 12 suspected signals were detected,involving cervix uteri ,fallopian tube ,ovary,pelvis cavity,uterus,vagina,urinary tract ,etc. Among them ,there were 11 positive signals of rivaroxaban ,and the bleeding events were concentrated in vaginal hematoma [ROR =12.07, 药。95%CI(8.51,17.12)],postmenopausal hemorrhage [ROR = 9.89,95%CI(8.31,11.77)],pelvic hematoma [ROR =7.68,95%CI(5.66,10.43)]. There were 4,4 and 2 suspicious signals for dabigatran etexilate ,apixaban and edoxaban. The main bleeding events of both apixaban [ ROR=5.18,95%CI(1.81,5.85)] and edoxaban [ROR =48.19,95%CI(6.76,343.77)] were vaginal hematoma ;dabigatran etexilate-induced pelvic hematoma [ROR = 12.56,95%CI(8.92,17.70)] had the strongest signal ,followed by urinary tract bleeding [ROR =5.41,95%CI(3.34,8.76)] and pelvic hemorrhage [ ROR=2.53,95%CI(1.88,3.40)]. CONCLUSIONS :Totally 4 kinds of NOACs can cause abnormal female reproductive system haemorrhage ,and the incidence of SAE is high ,of requiring hospitalization or prolonging hospitalization time. The risk of haemorrhage in rivaroxaban is the highest ,usually manifesting as vaginal hematoma ,postmenopausal hemorrhage and pelvic hematoma. Dabigatran etexilate mainly induce pelvic hematoma ,while apixaban and edoxaban are mainly cause vaginal hematoma.

ABSTRACT

@#Traditional surgical aortic valve replacement is associated with a high risk of serious complications, especially in elderly patients with other preoperative diseases and unable to undergo thoracotomy. Therefore, transcatheter aortic valve implantation (TAVI) is now the accepted standard treatment for patients with symptomatic severe aortic stenosis at elevated risk for conventional surgical valve replacement. Currently, guidelines propose the use of dual antiplatelet therapy for the prevention of thromboembolic events after TAVI in the patients without an indication for oral anticoagulation. While, this strategy is empiric and largely based on expert consensus extrapolated from the arena of percutaneous coronary intervention. Antithrombotic therapy is associated with a significant occurrence of both thrombotic and bleeding complications, thus, the balance between thrombotic and bleeding risk is critical. This review summarizes current guidelines and the evidence underpinning them and explores the rational for using antiplatelet and/or anticoagulant strategies after TAVI.

ABSTRACT

Venous thromboembolism (VTE) is one of the common complications of cancer and the second cause of death in cancer patients. Low molecular weight heparin(LMWH) is the standard of care but the high cost and the inconvenience of daily injections have led to low persistence with therapy. New oral anticoagulants (NOACs) have recently emerged as a new therapeutic option due to the ease of administration, especially compared to warfarin. The patients do not have to do laboratory monitoring for NOACs and there is less food or drug interactions. Several large randomized clinical trials have been performed and indicated that specific NOACs are more effective in the treatment of cancer-associated VTE compared with LMWH, but might increase the risk of bleeding. In some kinds of tumors, NOACs will be the reasonable alternative agents in the management of cancer-associated VTE. This review summarizes the current evidences for NOACs in the prevention and treatment of CAT, and there are ongoing studies providing more evidences for NOACs in CAT as well.

La infección por VIH ha sido reconocida en la actualidad como una condición protrombótica, reportándose en pacientes con SIDA 10 veces mayor riesgo de desarrollo de eventos tromboembólicos de los sistemas arterial y venoso; la literatura reciente ha estimado una incidencia entre el 0.26% y 7.6 %; dicha incidencia se describe asociada a infecciones. oportunistas y/o malignidades. Una variedad de mecanismos potenciales relacionados con factores del hospedador, el virus y el TARV (tratamiento antiretroviral), están implicados en el estado de hipercoagulabilidad en pacientes con SIDA, cuya fisiopatología conduce a alteraciones en la respuesta inflamatoria y de la cascada de coagulación que no han sido bien dilucidadas; sin embargo, incluye la presencia de anticuerpos anticardiolipinas, disminución de anticoagulantes naturales, y otros factores independientes que en conjunto originan morbi-mortalidad creciente si dicha predisposición a eventos trombóticos no es reconocida por el médico. Los autores de este artículo describen 2 casos de eventos trombóticos en pacientes con síndrome de inmunodeficiencia humana hospitalizados en el Servicio de Medicina Interna de Hospital Universitario de Caracas(AU)


AIDS infection has now been recognized as a prothrombotic condition reported in patients with this disease, and have been found 10 times at greater risk of developing thromboembolic events with systemic involvement. Recent literature has estimated an incidence between 0.26% and 7.6%, also described in association with opportunistic infections and / or malignancies. A variety of potential mechanisms related to host factors, virus and antiretroviral therapy (ART) are implicated in the hypercoagulability status in HIV-positive patients, the pathophysiology leading to alterations in the inflammatory response and the coagulation cascade. It has not been well understood and includes the presence of anticardiolipin antibodies, reduction of natural anticoagulants, and other independent factors that lead to increased morbidity and mortality if such predisposition to thrombotic events is not recognized by the clinician. The authors of this article describe 2 cases of thrombotic events in patients with human immunodeficiency syndrome hospitalized in the Hospital Universitario of Caracas, Venezuela(AU)

ABSTRACT

Venous thromboembolism includes 2 inter-related conditions: Deep venous thrombosis and pulmonary embolism. Heparin and low-molecular-weight heparin followed by oral anticoagulation with vitamin K agonists is the first line and current accepted standard therapy with good efficacy. However, this therapeutic strategy has many limitations including the significant risk of bleeding and drug, food and disease interactions that require frequent monitoring. Dabigatran, rivaroxaban, apixaban, and edoxaban are the novel oral anticoagulants that are available for use in stroke prevention in atrial fibrillation and for the treatment and prevention of venous thromboembolism (HYPERLINK\l "1). Recent prospective randomized trials comparing the NOACs with warfarin have shown similar efficacy between the treatment strategies but fewer bleeding episodes with the NOACs. This paper presents an evidence-based review describing the efficacy and safety of the new anticoagulants compared to warfarin.

ABSTRACT

A 91 year-old male who was orally receiving dabigatran and digitalis for chronic arterial fibrillation was hospitalized for cerebral infarction (CI). The complications of anemia due to gastrointestinal bleeding and the development of renal failure were also found, which enhanced the effect of digitalis. This enhanced drug action induced bradycardia and hypotension. The hypotension was improved by implanting a temporary pacemaker, and the neurological abnormal findings also disappeared with the improved blood pressure. In many cases, dabigatran is efficient in preventing CI, but in our case it in fact triggered a CI hemodynamically due to bradycardia and hypotension with gastrointestinal bleeding.
